Effective verbal and written communication abilities. Demonstrated ability to work independently and manage multiple tasks remotely.

Complete thorough medicines reconciliations using clinical correspondence. Identify and resolve discrepancies between current medication records and updated hospital communications. Review and document medication changes, initiating appropriate actions or flagging for GP follow-up. Collaborate remotely with GPs, care coordinators, and practice teams. Use clinical systems and decision-support tools to check for potential drug interactions, duplications, or contraindications. Maintain accurate clinical documentation in line with practice protocols and professional standards. Engage in ongoing professional development.
