Collaborate to assess client needs and develop community outreach plans Identify engagement opportunities with local tables and forums Participate in relevant forums and committees Foster a collaborative team environment Communicate regularly with collaborators and community partners Prepare and present reports to stakeholders Act as the key contact for community providers and resolve queries within 48 hours
