Remote RN Case Manager

Licensed Registered Nurse in good standing in the state of PA. 3+ years of RN Case Management experience in outpatient setting. CCM certification preferred, must obtain CCM within 1 year of employment. Supervisory experience preferred. Excellent written and oral communication skills. Excellent computer skills, including ability to learn multiple EMR and Case Management documentation systems. Ability to meet accreditation and quality standards (e.g., NCQA and HEDIS).

Partner with clinical teams to provide complex case management. Assist patients in achieving their highest level of wellness. Develop, implement, and maintain individualized care plans. Provide evidence-based clinical interventions. Partner with patient navigators to manage non-clinical care plan elements. Provide ongoing outreach to patients and monitor patient data. Develop patient-specific escalation plans. Provide education regarding conditions and treatment options. Serve as an advocate for the patient. Follow up on service or resource referrals. Maintain up-to-date documentation in the electronic medical health record.
