Requirements
- Hold a Doctor of Pharmacy (PharmD) or equivalent advanced degree.
- Be a licensed pharmacist in the state of Florida.
- Have knowledge of specialty medication pharmacology and local, state, and federal pharmacy regulations.
- Have strong clinical and technical skills and the ability to manage processes and maintain legal compliance.
- Be proficient with Microsoft Office tools, including Outlook, Teams, Word, Excel, and PowerPoint, and with database and reporting tools.
- Be able to identify barriers to medication adherence and implement and monitor plans to improve adherence.
- Be able to motivate and coach patients and identify therapy goals.
- Have knowledge of complex or chronic disease states and specialty pharmacy therapies; experience in the listed specialties is preferred.
- Have 1+ years of experience; this is strongly preferred but not required.
- Knowledge of EPIC, QS1, or WILLOW software is a plus.
Responsibilities
- Review clinician patient-care notes and assessments to support prior authorization approvals and care plan recommendations.
- Develop and refine disease- and drug-specific clinical protocols to support medication adherence and evidence-based care.
- Monitor patient adherence using PDC reports, patient assessment data, and mobile platform data, with a focus on oncology and other specialty disease states.
- Evaluate EMR records for drug and disease interactions, medication selection, labs and vitals, allergies, prescriptions, and other information relevant to medication adherence and patient outcomes.
- Recommend alternative drug regimens and disease-specific treatments according to guidelines, and evaluate therapies for side effects or other medication-related issues.
- Provide medication therapy management, document interventions, assess adherence, and conduct clinical interventions such as medication reconciliation, coaching, and regimen optimization.
- Oversee technicians' daily activities and verify delivery coordination slips, prior authorizations, and ongoing prior authorization activity.
- Identify and resolve insurance issues and recognize opportunities for financial assistance.
- Communicate with prescribers and other health professionals, and supervise support personnel communication in compliance with HIPAA rules and regulations.
- Monitor productivity, manage relevant data, participate in process improvement, and support staff training and research projects.
