Requirements
- Hold an active pharmacist license.
- Have a Doctor of Pharmacy (PharmD) degree from an accredited School of Pharmacy.
- Have at least 5 years of clinical pharmacy experience involving direct collaboration with physicians and other healthcare providers in hospital or ambulatory care settings.
- Understand CMS Star measures, HEDIS measures, MIPS, and National Quality Forum measures.
- Demonstrate excellent written and verbal communication skills for explaining clinical information to diverse audiences.
- Bring strong organizational skills, attention to detail, and the ability to manage multiple priorities.
- Demonstrate analytical, strategic, and problem-solving capabilities with a metrics-driven approach.
- Work independently and collaborate effectively across multidisciplinary teams.
- Be proficient with Microsoft Office applications, including Outlook, Word, Excel, PowerPoint, and Teams.
- Be able to work remotely and perform computer-based tasks involving extended sitting and repetitive typing or texting.
- Renal patient pharmacy management and value-based care experience are preferred.
- Experience with health plans or healthcare vendors and ASHP-accredited pharmacy residency experience are preferred.
Responsibilities
- Review medication regimens and recommend adjustments using clinical protocols, guidelines, and evidence-based practices.
- Develop educational resources for providers, members, and staff on medications, indications, guideline updates, and clinical information.
- Contribute to treatment algorithms, clinical guidelines, policies, procedures, and disease management initiatives.
- Respond to pharmacy referrals involving medication access, affordability, education, adherence, optimization, and safety.
- Evaluate high-cost, high-risk, and high-volume medications using a population health approach.
- Improve medication reconciliation and adherence processes during transitions of care and chronic disease management.
- Collaborate with physicians, care managers, and other healthcare professionals to support coordinated care.
- Oversee pharmacy technicians and provide direction and support.
- Participate in quality improvement, process enhancement, and other pharmacy or clinical initiatives.
- Maintain compliance with applicable policies, procedures, regulations, and patient health information protections.
