Requirements
- Doctor of Pharmacy (PharmD) degree.
- Active pharmacist license in Washington.
- Completion of a PGY2 Ambulatory Care Residency OR PGY1 Residency with 1 year of ambulatory practice under a CPA OR 3 years of ambulatory practice with disease state management.
- Proficiency in Microsoft Office tools (Outlook, Teams, Word, Excel, and PowerPoint).
- Ability to navigate the internet and web-based platforms.
- Strong communication, organization, and technical skills.
- Empathy, integrity, and a team-first mindset.
- Ability to pass a pre-employment data entry and computer skills assessment.
Responsibilities
- Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug‑level assessments.
- Partner with physicians and interdisciplinary teams to order individualized care plans under a Collaborative Practice Agreement (CPA).
- Monitor therapy effectiveness and safety, identifying optimization opportunities and intervening as appropriate.
- Support quality‑improvement and REMS initiatives.
- Provide operational support for complex prior authorizations and improve workflows.
- Educate patients and maintain ongoing clinical development.
