Requirements
- High school diploma or GED required; a bachelor’s degree is preferred.
- At least 2 years of experience in health information management, medical records, healthcare compliance, revenue cycle support, or related healthcare operations.
- Demonstrated experience working with electronic health record (EHR/EMR) systems.
- Experience processing release of information requests is preferred.
- Working knowledge of HIPAA privacy and security requirements, medical record standards, documentation practices, and health information management principles.
- Understanding of healthcare regulatory and accreditation requirements.
- Proficiency with Microsoft Office applications and electronic health record systems.
- Ability to analyze records, identify deficiencies, and recommend or implement corrective actions.
- Strong attention to detail, accuracy, proofreading, organization, and time management.
- Ability to manage sensitive information with discretion, work independently, manage priorities, and meet deadlines.
- Commitment to maintaining a remote work schedule aligned with Pacific Time business hours.
- Preferred professional certifications include RHIT, RHIA, CHPS, or CPHIMS.
Responsibilities
- Maintain and monitor electronic and paper medical records for completeness, accuracy, accessibility, and compliance.
- Review records for missing documentation, deficiencies, and signatures, and coordinate corrections or amendments.
- Support designated record set management and maintenance of the legal medical record.
- Conduct medical record quality reviews and audits and assist with corrective actions.
- Process release of information requests and verify identities, authorizations, and legal requirements before disclosing protected health information.
- Coordinate responses to subpoenas, audits, investigations, payer requests, and regulatory inquiries while documenting disclosures.
- Monitor compliance with HIPAA, privacy requirements, retention standards, accreditation expectations, and internal policies.
- Monitor EHR/EMR data quality and documentation standards, and collaborate with teams on workflow testing and optimization.
- Identify documentation trends and recommend process and quality improvements.
- Guide staff on medical record requirements, release procedures, privacy standards, and documentation practices.
