Requirements
- Active, unrestricted RN license with the ability to obtain California licensure within 6 months of hire.
- Willingness to obtain additional state licensure as needed.
- 2 years of people management experience (4 years preferred).
- 2-3 years of care management experience.
- 1-2 years of post-acute experience.
- Willingness to work Monday through Friday 8:30am-5:00pm Pacific Time with flexibility for potential evenings and weekends.
- Proficiency in Microsoft and Google applications.
- Proven results-driven leadership with change management experience in rapid-growth environments.
- Strong organizational and independent working skills.
- Excellent communication and motivational skills, including conflict resolution techniques.
Responsibilities
- Supervise a team of approximately 10-15 direct reports to ensure high-quality enrollments and care management services.
- Oversee client outreach, enrollments, and the creation of Individual Care Plans.
- Serve as an escalation point for complex clinical cases.
- Drive the team to meet key performance indicators (KPIs).
- Collaborate with the Director(s) of Enhanced Care Management to implement process improvements.
- Provide clinical oversight, including clinical sign-offs and leading Interdisciplinary Care Team (IDT) meetings.
- Provide direct care management as needed to support organizational goals.
