Requirements
- Active, unrestricted LCSW or LMFT license in California required.
- Willingness to obtain additional state licensure as needed.
- 2 years of people management experience (4 years preferred).
- 2-3 years of care management experience.
- 1-2 years of post-acute experience.
- Available to work Monday through Friday 8:30am-5:00pm Pacific Time.
- Ability to travel up to 20-25% for training, meetings, and business needs.
- Proficient in Microsoft and Google applications.
- Strong organizational and independent working skills.
- Proven experience in change management in rapid-growth environments.
Responsibilities
- Supervise a team of approximately 10-15 direct reports to ensure high-quality enrollments and care management services.
- Oversee client outreach, enrollments, and the creation of Individual Care Plans.
- Serve as an escalation point for complex clinical cases and manage staff performance.
- Drive the team to meet key performance indicators (KPIs) effectively.
- Collaborate with the Director(s) of Enhanced Care Management to implement changes and process improvements.
- Provide clinical oversight including clinical sign-offs, oversight of care plans, and leading IDT meetings.
- Provide direct care management as needed to support organizational goals.
