Requirements
- Strong background in athenaOne (athenahealth EMR).
- Experience working in primary care or internal medicine clinics.
- Experience working with older adult patients.
- Excellent communication skills.
- Patience and ability to adapt communication for patients with memory or cognitive challenges.
- Ability to communicate clearly with English-speaking patients.
Responsibilities
- Manually migrate patient data into athenaOne and prepare charts ahead of visits.
- Conduct patient outreach for appointment follow-up, reminders, and care coordination.
- Gather care goals, medical records, and medication lists from wait-listed patients.
- Assist with prior authorizations and manage pending prescription refills.
- Manage patient calls, secure messages, and callbacks.
- Triage phone calls for potentially urgent concerns using physician-provided templates.
- Perform medication reconciliation and coordinate specialist referrals.
