Requirements
- 1-2 years of experience in home health intake, referrals, admissions, or a similar healthcare setting.
- Familiarity with home health orders, insurance verification, and physician coordination.
- Experience using EMR or referral-management systems.
- Strong communication, organization, and attention to detail.
- Ability to manage multiple referrals and competing priorities.
- Working knowledge of Medicare home health guidelines and OASIS requirements preferred.
- Comfortable identifying problems and escalating concerns.
Responsibilities
- Manage the full intake workflow from referral through admission and scheduling.
- Process and enter referrals accurately into Axxess EMR.
- Review incoming referrals for completeness and clinical documentation accuracy.
- Verify insurance coverage and obtain required home health authorizations.
- Coordinate with hospitals, physician offices, case managers, and insurance companies.
- Schedule clinicians based on discipline, availability, and patient needs.
- Maintain documentation and track open referrals until resolution.
- Provide referral status updates to clinical and scheduling teams.
