Requirements
- Master's degree in Social Work from a CSWE-accredited program.
- Valid and current master’s level clinical license (e.g., LCSW, LISW).
- 3-5 years of social work experience in a healthcare setting.
- Experience in oncology, palliative care, or hospice care.
- Ability to obtain additional state licenses as needed.
- Comfort using technology, including video chat, Google Suite, and EHR systems.
- Exceptional organizational skills for documentation and multitasking.
- Strong communication and rapport-building skills.
Responsibilities
- Conduct comprehensive psychosocial assessments to address emotional concerns and financial/caregiver needs.
- Provide immediate support and intervention for behavioral health crises and prognosis changes.
- Connect patients and families with relevant community resources for social or behavioral health needs.
- Coordinate care with multidisciplinary teams including oncologists, nurses, and behavioral health providers.
- Advocate for patient treatment access with insurance companies and healthcare providers.
- Facilitate support groups and provide education on cancer-related topics.
