Requirements
- Active California Licensed Vocational Nurse (LVN) license, Medical Assistant certificate, or Bachelor’s degree in a related field.
- Minimum of 2 years of telephonic outreach or enrollment experience in a high call volume environment.
- Experience working with complex care clients and ECM Populations of Focus.
- Available to work Monday through Friday 8:30am-5:00pm Pacific Time.
- Comfortable with technology, including video conferencing, Google Workspace, Slack, and electronic health records.
- Excellent oral and written communication skills and positive interpersonal skills.
- Prior experience in a remote role and ability to maintain a quiet, secure workspace for privacy.
- Bilingual (English/Spanish) skills are highly preferred.
- Experience with Athena Health and Health Gorilla is a plus.
Responsibilities
- Engage with prospective clients via 60-100 outbound calls daily and document interactions in the CRM.
- Review medical charts to document client needs and recommend appropriate clinical interventions.
- Convey client information and preferences to the care team to support personalized care planning.
- Assist clients in completing psychosocial assessments and defining actionable care goals.
- Apply evidence-based practices, including Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
- Participate in weekly team operations meetings to coordinate and optimize care delivery.
