Requirements
- Certified as a Nurse Practitioner.
- Active state licensure in Texas (TX).
- 5+ years of advanced clinical experience in complex care, chronic care management, or primary care.
- Fluency in medical Spanish, both verbal and written.
- Highly comfortable using technology to deliver virtual care.
- Ability to obtain additional state licensures.
- Experience or strong interest in digital-first healthcare and virtual medicine.
- Availability to work at least two 4-hour shifts each weekend in addition to weekday hours.
- Commitment to a minimum of 14 hours per week.
Responsibilities
- Provide best-in-class care for patients managing complex and chronic illnesses using proprietary technology.
- Deliver compassionate and empathetic care to patients of all ages, including those with diabetes, hypertension, obesity, and mental health conditions.
- Collaborate with clinical team members to solve patient problems efficiently and resourcefully.
- Utilize asynchronous text-based chat and synchronous video for patient communication.
- Engage in shared decision-making with patients to optimize outcomes.
