Requirements
- Active Texas Nurse Practitioner license.
- 1+ year of professional experience in Family Medicine or Internal Medicine.
- Experience working with Medicare and Medicaid populations.
- Demonstrated experience with HEDIS and risk adjustment workflows.
- Completion of Medicare and Medicaid provider enrollment.
- Comfortable delivering patient care through telehealth platforms.
- Ability to consistently work weekday/daytime hours.
- Multi-state NP licenses preferred.
- Value-based care model experience preferred.
- Annual Wellness Visit experience preferred.
- HCC risk adjustment documentation experience preferred.
Responsibilities
- Conduct comprehensive health assessments via telehealth platforms.
- Document risk adjustment (HCC coding) during patient visits.
- Close HEDIS care gaps during patient encounters.
- Review medical history, medications, and preventive care needs.
- Document visits accurately using ICD-10 and CPT II codes.
- Deliver clear care plans and provide follow-up guidance to patients.
- Maintain high-quality visit documentation for quality programs.
