Requirements
- Active and unrestricted New York State Registered Nurse (RN) license.
- Minimum one (1) year of home care experience.
- Knowledge of New York State Department of Health requirements and regulations.
- Proficiency with electronic medical record or workflow tracking systems.
- Strong clinical assessment and documentation review skills.
- Ability to interpret regulatory and quality standards.
- High attention to detail and ability to manage multiple deadlines.
- Strong written and verbal communication skills for clinical coaching.
- Ability to work independently in a hybrid environment.
- Commitment to HIPAA and patient confidentiality.
Responsibilities
- Review clinical documentation including Start of Care Assessments, Reassessments, and Care Plans for accuracy and compliance.
- Collaborate with field nurses to resolve documentation deficiencies, missing items, or noncompliant records.
- Manage documentation status and workflows within Medflyt to meet submission deadlines.
- Submit completed documentation for physician review and signature.
- Conduct quality assurance audits and maintain tracking reports for clinical leadership.
- Provide coaching and education to clinical staff on documentation standards and best practices.
- Conduct occasional patient home visits to support agency or patient care needs.
