Requirements
- Current CMA or CCMA certificate issued by a recognized credentialing body.
- 2 years of healthcare experience.
- 1 year of telephonic or call center customer service experience.
- Experience with EHR platforms for documentation and scheduling.
- Training in motivational interviewing and patient education.
- Comfort discussing serious illness, chronic disease, and end-of-life topics.
- Proficient in Microsoft and Google applications.
- Access to a reliable internet connection.
- Strong written and verbal communication skills.
- Willingness to work Monday through Friday, 8:30am–5pm EST.
Responsibilities
- Conduct targeted telephonic outreach to patients to educate them on palliative care and VyncaCare's services.
- Complete structured screening forms to determine program eligibility based on chronic disease history and functional decline.
- Schedule eligible patients for initial visits with a nurse practitioner.
- Act as a liaison between patients and the care team, responding to questions and coordinating logistics.
- Maintain accurate patient documentation in compliance with HIPAA standards.
- Collaborate with business development teams to identify outreach trends and barriers to improve program reach.
