Requirements
- Bachelor's degree in a clinical or healthcare-related discipline, or a verified equivalent in professional experience.
- Active nursing credential as a Registered Nurse (RN), Licensed Practical Nurse (LPN), or recognition as an International Medical Graduate (IMG).
- Minimum of 5+ years of clinical experience within a complex healthcare environment.
- Minimum of 6+ years of professional experience operating within a value-based care landscape (ACO, MCO, or large-scale health system).
- Demonstrated competency in exercising high levels of autonomy and managing executive-level stakeholder relationships.
- Advanced proficiency in architecting and delivering data-driven collateral to drive change management.
- Direct experience collaborating with post-acute care providers and health system personnel.
- Superior public speaking and facilitation capabilities.
- Ability to travel up to 25%.
Responsibilities
- Serve as the principal post-acute authority and centralized liaison for the flagship Enterprise Practice.
- Function as an executive-level Account Manager, fostering deep-seated partnerships with practice leadership and hospital-based population health navigators.
- Spearhead post-acute initiatives ensuring alignment with organizational strategic objectives.
- Develop and present sophisticated collateral synthesizing claims data and clinical insights into operational workflows.
- Drive improvements in patient wellness metrics such as length of stay and readmission rates.
- Orchestrate strategic quarterly Town Halls with SNFs, hospitals, and practice leads.
- Architect and refine specialized protocols for SNF patient management.
