- Minimum of two years’ management or supervisory experience in home health, managed care, or long-term care settings.
Requirements
- Master’s Degree in Social Work.
- Valid New York State LCSW license and registration.
- Minimum of two years of management or supervisory experience in home health, managed care, or long-term care settings.
- At least one year of experience working with elderly or frail populations.
- Ability to pass medical clearance and immunizations as per organizational policy.
- Strong customer service orientation.
- Excellent organizational and communication skills.
Responsibilities
- Review comprehensive assessments and care plans to guide field staff on quality documentation and interventions.
- Represent the social work discipline in interdisciplinary care planning meetings.
- Coordinate essential services including ADLs, IADLs, housing, transportation, and community resources.
- Monitor and evaluate care activities to adjust plans based on evolving health needs.
- Provide ongoing training, performance feedback, and quality improvement initiatives for field staff.
- Ensure all work complies with regulatory standards and organizational policies.
