Requirements
- 5+ years of experience treating patients with chronic diseases (CHF, Hypertension, T2D) in outpatient or inpatient settings.
- Experience managing CHF with guideline-directed medical therapy (GDMT).
- Active Master’s Degree as a Nurse Practitioner.
- Active ANCC or AANP certification.
- Active Compact Multi-State RN license.
- Active NP licensure in at least two of the following states: TX, MI, NC, CA, PA, NJ, CT, AL.
- Home office setup with consistently stable Wi-Fi and the ability to pass a speed test.
- Experience working in a CHF bridge clinic, T2D clinic, or startup environment preferred.
- Familiarity with remote patient monitoring technology.
- Comfort using AI-assisted clinical tools while maintaining independent clinical judgment.
Responsibilities
- Oversee the monitoring of patient vitals, symptoms, and labs in collaboration with RNs and MDs.
- Manage treatment plans with an emphasis on initiating and titrating guideline-directed medical therapy (GDMT) for Heart Failure, Hypertension, and Type 2 diabetes.
- Respond to patient escalations identified via Remote Patient Monitoring (RPM).
- Lead regular virtual check-ins with patients to review labs, GDMT optimization, and adherence.
- Collaborate with the patient’s physician to optimize treatment plans and determine appropriate levels of care.
- Support internal growth efforts to scale care delivery for chronic conditions.
- Shape the culture and operations of the clinical team.
