Requirements
- Bachelor's Degree in Pharmacy or Doctor of Pharmacy (PharmD) from an ACPE-accredited institution or equivalent international pharmacy education.
- Current pharmacist license in the practicing state(s), with the ability to obtain additional state licenses if required.
- Completion of a Medication Therapy Management (MTM) certificate program.
- Minimum of 1 year of professional pharmacist experience.
- Strong clinical knowledge in medication therapy management, chronic disease management, medication safety, adherence strategies, and evidence-based therapeutic decision-making.
- Experience communicating effectively with patients, caregivers, prescribers, and multidisciplinary healthcare teams.
- Excellent verbal and written communication skills, including the ability to explain complex medical information clearly and compassionately.
- Strong organizational skills, attention to detail, and the ability to work independently in a remote environment while managing multiple priorities.
- Proficiency with electronic clinical systems, Microsoft Office applications, and telehealth technologies.
Responsibilities
- Deliver comprehensive Medication Therapy Management (MTM) services, including Comprehensive Medication Reviews, Targeted Medication Reviews, adherence interventions, cost-saving recommendations, and transitions of care support.
- Conduct proactive outreach to patients, explain the benefits of MTM services, address concerns, and encourage participation using effective communication and motivational techniques.
- Respond to inbound patient inquiries, assess medication-related needs, and provide appropriate clinical guidance in accordance with established protocols.
- Evaluate medication regimens for appropriateness, effectiveness, safety, adherence, dosing accuracy, and potential therapy gaps while developing evidence-based recommendations.
- Collaborate with prescribers and interdisciplinary healthcare teams by communicating clinical interventions and documenting recommendations to support coordinated patient care.
- Educate patients and caregivers on chronic disease management, medication adherence, preventive care, and treatment plans to improve long-term health outcomes.
- Maintain accurate, timely, and compliant clinical documentation while adhering to applicable pharmacy regulations, CMS requirements, HIPAA standards, and internal quality guidelines.
