- Minimum of 3 years of experience in a clinical setting, preferably with a focus on senior care.
- Strong assessment and care planning skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage a caseload effectively.
- Proficiency with technology and ability to conduct virtual consultations.
- Compassionate and empathetic attitude towards seniors and their families.
- Willingness to travel for occasional in-person visits with clients (if needed).
- Availability to work full-time, Monday-Friday during regular business hours.
Responsibilities
- Conduct comprehensive assessments of clients' health care needs and develop individualized care plans.
- Provide ongoing monitoring and evaluation of clients' health status and make necessary adjustments to care plans.
- Educate clients and their families on disease management, medication management, and other health-related topics.
- Collaborate with other health care professionals, such as physicians and therapists, to ensure coordinated and comprehensive care.
- Serve as a resource and advocate for clients and their families, addressing any concerns or questions they may have.
- Utilize telehealth technology to conduct virtual consultations with clients and their families.
- Document all client interactions and maintain accurate and up-to-date records.
