Requirements

  • Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credentialed by the Commission on Dietetic Registration.
  • Active state licensure/certification where required to practice.
  • Proven experience caring for older adults.
  • Demonstrated skill as a coach, with a motivating and supportive approach.
  • Comfort and proficiency practicing via telehealth.
  • Ability to contribute to building new clinical protocols and patient journeys.
  • Commitment to the mission of improving health outcomes for older adults.
  • Ability to work collaboratively as part of a multidisciplinary clinical team.

Responsibilities

  • Partner with older adults to assess nutritional needs and build personalized, evidence-based nutrition care plans.
  • Deliver medical nutrition therapy and coaching grounded in lifestyle medicine with a whole-food, plant-based emphasis.
  • Support patients managing chronic conditions where nutrition is central, such as diabetes, cardiovascular and renal health, and metabolic health.
  • Anchor work in the 4 M's of Age-Friendly care—What Matters, Medication, Mentation, and Mobility.
  • Collaborate with care team members including NPs, behavioral health professionals, and primary care coordinators.
  • Help shape the clinical model, including protocols, intake processes, outcome measures, and patient experience.
