Requirements
- Active Compact RN license.
- 1 year of acute care nursing experience.
- ADN or Diploma in Nursing (BSN preferred).
- Home office with a locked door for HIPAA compliance.
- Primary and secondary workspace with 100Mbps download and 20Mbps upload speeds.
- Ability to work set schedules including every-other-weekend rotation.
- Ability to participate in on-call and rotating holiday requirements.
- Proficiency in clinical triage and critical thinking.
- Effective communication, teamwork, and conflict resolution skills.
- Ability to work independently in a remote environment.
Responsibilities
- Provide telephonic triage and virtual monitoring to assist callers.
- Offer care recommendations, education, and advice using decision-making tools and clinical judgment.
- Partner with customers to reduce readmissions and manage chronic diseases.
- Schedule provider appointments and facilitate communications.
- Document patient encounters accurately in the EMR.
- Escalate identified needs to support services.
- Participate in process improvement and peer review activities.
