Requirements

  • Active Compact RN license.
  • 1 year of acute care nursing experience.
  • ADN or Diploma in Nursing (BSN preferred).
  • Home office with a locked door for HIPAA compliance.
  • Primary and secondary workspace with 100Mbps download and 20Mbps upload speeds.
  • Ability to work set schedules including every-other-weekend rotation.
  • Ability to participate in on-call and rotating holiday requirements.
  • Proficiency in clinical triage and critical thinking.
  • Effective communication, teamwork, and conflict resolution skills.
  • Ability to work independently in a remote environment.

Responsibilities

  • Provide telephonic triage and virtual monitoring to assist callers.
  • Offer care recommendations, education, and advice using decision-making tools and clinical judgment.
  • Partner with customers to reduce readmissions and manage chronic diseases.
  • Schedule provider appointments and facilitate communications.
  • Document patient encounters accurately in the EMR.
  • Escalate identified needs to support services.
  • Participate in process improvement and peer review activities.
