Requirements
- Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or a closely related field (or 4 years of commensurate experience).
- At least 2 years of experience in health care, preferably in care coordination.
- At least 1 year of experience serving adults with intellectual/physical disabilities or older adults.
- Valid and unrestricted driver's license with reliable transportation and adequate auto insurance.
- Demonstrated knowledge of long-term care programs and principles of self-determination.
- Proficiency in Microsoft Office suite and ability to learn applicable software programs.
- Ability to work independently with minimal supervision and manage multiple projects simultaneously.
- Excellent time-management, prioritization, and critical-thinking skills.
- Strong verbal and written communication skills.
- Ability to adapt style to work effectively with diverse populations and in various settings.
Responsibilities
- Conduct regularly scheduled in-person visits with IRIS participants in their homes and community, including a minimum of four meetings per year.
- Provide program orientation to new participants and explain their rights, responsibilities, and use of public funds.
- Develop customized IRIS plans focused on employment, housing, health, safety, community membership, and personal relationships.
- Assist participants in managing their budgets, interpreting monthly reports, and navigating provider billing and risk mitigation.
- Document all orientation, planning, and modification activities in the WISITs data system within 48 business hours.
- Research and connect participants to community resources and natural supports that align with their personal outcomes.
- Attend monthly in-person team meetings and weekly check-ins with the IRIS Consultant Supervisor.
- Maintain confidentiality and ensure compliance with HIPAA regulations.
