Requirements
- Bachelor's degree in social work, psychology, human services, counseling, nursing, special education, or closely related field (or 4 years of equivalent experience).
- At least 2 years of experience in health care, preferably in care coordination.
- At least 1 year of experience serving adults with disabilities or older adults.
- Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance.
- Demonstrated knowledge of long-term care programs and principles of self-determination.
- Proficiency with Microsoft Office suite and applicable software programs.
- Strong time-management, critical-thinking, and problem-solving skills.
- Ability to work independently with minimal supervision and adapt to changing environments.
- Excellent verbal and written communication skills.
- Ability to maintain confidentiality and HIPAA compliance.
Responsibilities
- Provide program orientation to new participants and educate them on rights and responsibilities.
- Collaborate with participants to identify long-term care goals related to housing, health, and community membership.
- Develop and maintain customized IRIS plans and manage associated participant budgets.
- Research and connect participants with local community resources and natural supports.
- Document all planning activities and budget modifications within the WISITs data system.
- Conduct regular in-person home and community visits with participants.
- Liaise between the Fiscal Employer Agency and participants to support billing and payment tracking.
- Attend monthly in-person team meetings and participate in weekly supervisor check-ins.
