Responsibilities
- Conduct proactive outreach to at-risk students and families to address academic or personal concerns.
- Develop and guide individualized intervention and support plans.
- Facilitate Zoom meetings and support sessions for students and teachers.
- Maintain accurate documentation of student records, support plans, and intervention data.
- Collaborate with internal teams, teachers, and administrators to ensure strategic support.
- Track student attendance and MTSS-related participation data.
- Provide regular follow-up to monitor student progress and adjust interventions as needed.
