Responsibilities
- Review case load and prepare technology for daily patient services.
- Navigate daily schedule and partner with in-home Sprinter clinical team members.
- Build connections with patients to collect information and perform services.
- Conduct thorough health risk assessments, medication reviews, and cognitive screenings.
- Empower patients with health and well-being educational information.
- Evaluate vital signs, arrange blood draws, and coordinate necessary tasks.
- Provide comprehensive care coordination and preventive care management.
- Manage medications and facilitate referrals to specialists and community resources.
- Document all findings according to high-quality standards.
