Requirements
- MD or DO degree.
- Active, unrestricted licensure in PA, CA, AZ, TX, SC, NM, NJ, and TN.
- Board certified in Family Medicine, Internal Medicine, or primary care specialty.
- Proven experience managing or providing oversight to other clinicians.
- Deep understanding of community health and Social Determinants of Health.
- Experience with telehealth platforms.
- Comfort working with Medicare and Medicaid populations.
- Proficiency with EHR systems, Slack, and Google Suite.
- Strong communication skills for virtual team management.
Responsibilities
- Conduct initial telehealth visits for members to assess medical needs and care gaps.
- Build, mentor, and oversee a high-performing clinical team.
- Perform clinical oversight and quality assurance audits on documentation.
- Onboard and train new physicians on clinical workflows and company standards.
- Refine clinical policies, intake forms, and care plan templates.
- Track and report on clinical KPIs including ER diversion and SDoH resolution.
- Collaborate with care navigators and social workers to coordinate care.
