Active Family Nurse Practitioner license in Louisiana
Technologically savvy with experience using laptops and mobile devices
Experience using HIPAA secure messaging apps

Pass national background check
Pass valid clinical license search

Responsibilities

Review case load and prepare technology for virtual visits
Partner with in-home team members to coordinate patient care
Conduct virtual adult and/or pediatric wellness visits
Perform health risk assessments and cognitive screenings
Manage patient medications and perform reviews
Evaluate vital signs and arrange blood draws
Provide preventive care interventions and refer to specialists
