Requirements
- Active compact multi-state LPN/LVN license in the state where you reside.
- Active CA state LVN license.
- Minimum 5 years of clinical experience as an LPN.
- Experience with patient education and chronic condition management.
- Proficiency with AI-assisted clinical tools.
- Comfortable working independently in a remote, fast-paced environment.
- Stable high-speed WiFi.
- Excellent interpersonal communication skills.
Responsibilities
- Conduct outbound telephonic patient outreach and one-on-one coaching.
- Help patients execute personalized care plans and achieve goals.
- Conduct comprehensive assessments of health status, lifestyle, and readiness to change.
- Educate patients on disease management, medication adherence, and symptom recognition.
- Monitor progress and health outcomes through remote monitoring tools.
- Document patient interactions and outcomes in the Cadence platform.
- Collaborate in real time with clinicians and patient support teams.
