Requirements
- Active compact multi-state LPN/LVN license in the state of residence.
- Active CA state LVN license.
- At least 5 years of clinical experience as an LPN.
- Remote work readiness including stable high-speed WiFi.
- Experience in patient education and support for chronic conditions.
- Fluency with AI-assisted clinical tools.
- Ability to navigate and troubleshoot multiple systems simultaneously.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching.
- Help patients execute personalized care plans focusing on behavior modification and self-management.
- Conduct comprehensive health status assessments.
- Educate patients on disease management, medication adherence, and symptom recognition.
- Monitor patient progress and health outcomes through regular check-ins.
- Document interactions and identify barriers to care in the company platform.
- Collaborate with clinicians and patient support teams in real time.
