Requirements
- Active compact multi-state LPN/LVN license in state of residence.
- Active California state LVN license.
- At least 5 years of clinical experience as a Licensed Practical Nurse.
- Remote work readiness including stable high-speed WiFi.
- Experience providing patient education and support to individuals managing chronic conditions.
- Fluency with AI-assisted clinical tools (e.g., Gemini, Open Evidence).
- Ability to navigate and troubleshoot multiple systems simultaneously.
- Strong interpersonal communication skills for phone-based patient interaction.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic conditions.
- Help patients execute personalized care plans focusing on behavior modification and self-management.
- Conduct comprehensive assessments of health status, lifestyle, and readiness to change.
- Educate patients on disease management, medication adherence, and symptom recognition.
- Monitor progress and health outcomes through regular check-ins and remote monitoring tools.
- Document patient interactions and follow-up plans in the Cadence platform.
- Collaborate with clinicians and patient support teams to escalate concerns.
