Requirements
- Active RN license in good standing (willing to obtain licensure in other states).
- Minimum of 5 years of RN experience.
- Willingness to work PST hours with evening/weekend flexibility.
- Proficient technological skills including Microsoft products.
- Ability to travel occasionally for corporate gatherings and events.
- Experience developing care plans for individuals with chronic conditions preferred.
- BSN highly preferred.
- CDE or CDCES certification highly preferred.
- Experience working 100% remote as a nurse highly preferred.
Responsibilities
- Establish and maintain contact with assigned and active members via video, phone, text, and email.
- Obtain and utilize clinical information to develop an individualized member care plan and clinician-centered care plan per NCQA guidelines.
- Assess members health status and care coordination needs.
- Collaborate with members’ health care providers as needed.
- Utilize motivational interviewing techniques, individualized care plans, and the Triple Aim framework to empower active members.
- Explain program benefits to new or inactive members.
- Appropriately document and handle member information in accordance with HIPAA compliance.
- Meet individual performance metrics to support member engagement and retention.
