Responsibilities
- Educate potential patients and family members on the program and enroll them.
- Assist patients in navigating the program app and/or website.
- Initiate outbound calls and respond to inbound calls promptly.
- Assess patient eligibility based on provided documentation.
- Record and maintain accurate information and call notes in the system.
- Achieve weekly targets as a productivity measure.
- Ensure timely follow-up for interested patients.
- Stay informed about cardiac rehabilitation and attend training.
- Send educational material through email.
- Communicate insurance or other information to relevant parties.
