Requirements
- Degree in healthcare, social work, or related field (Bachelors preferred)
- 2+ years of experience in a healthcare, behavioral health, or administrative role
- Strong communication skills
- Proficiency with data entry and EHR/CRM systems
- Experience working in a remote fast-paced startup environment
- Dedicated quiet space with high-speed internet (min 100 Mbps download/20 Mbps upload)
- Experience in behavioral health settings (preferred)
- Familiarity with the Collaborative Care Model (preferred)
- Bilingual fluency in Spanish and English (preferred)
- Knowledge of medical terminology and patient intake processes (preferred)
Responsibilities
- Monitor multiple Electronic Health Record systems for new referrals and provider communication
- Serve as the first point of contact for referred patients
- Manage patient referral entry and outreach queue
- Educate patients about the Collaborative Care Model
- Contact disengaged and unreached patients
- Utilize de-escalation techniques for distressed patients
- Follow safety protocols for crisis intervention
- Communicate via phone, SMS, and email for enrollment
- Collaborate with healthcare providers and internal teams
- Assist patients with insurance verification
