Requirements
- Master’s degree in Counseling, Social Work, Psychology, or a related clinical discipline.
- Active and unrestricted clinical licensure in the State of Illinois.
- Strong experience in mental health and substance use assessment, treatment, and case management.
- Previous experience within Employee Assistance Programs (EAP), formal referrals, or fitness-for-duty evaluations preferred.
- Proven ability to manage sensitive and complex clinical situations independently and professionally.
- Excellent communication and interpersonal skills with the ability to engage senior-level executives and stakeholders.
- Strong organizational and documentation skills with experience using CRM or case management systems.
- Ability to work effectively in high-pressure environments while exercising sound clinical judgment.
- Experience consulting with healthcare organizations or workplace wellness programs is highly desirable.
- Strong collaboration, problem-solving, and relationship-building capabilities in remote work settings.
Responsibilities
- Serve as the primary clinical point of contact for formal referrals, assessments, and treatment coordination activities.
- Manage complex clinical cases from initial consultation through ongoing care management and follow-up support.
- Collaborate with treatment providers, referral partners, and stakeholders to ensure high-quality, client-centered care delivery.
- Participate in customer meetings and present case updates, recommendations, and program insights to key organizational stakeholders.
- Conduct initial consultations to assess referral needs and align care strategies among all involved parties.
- Coordinate releases of information and ensure compliance with confidentiality and documentation standards.
- Maintain timely, accurate, and organized case documentation within CRM and case management systems.
- Provide prompt responsiveness and support for acute cases, urgent referrals, and ongoing client needs.
- Participate in regular case review meetings and contribute clinical expertise to ongoing treatment planning discussions.
- Partner with leadership teams to maintain strong customer relationships and support service excellence initiatives.
