Requirements

- Proficiency with Outlook and Microsoft Office programs
- Ability to work Saturday-Wednesday schedule
- Pass drug screen and background check
Responsibilities
- Complete individualized patient care plans using electronic medical record system
- Perform care management and coordination services
- Maintain frequent contact with patients to support compliance and outcomes
- Document all client communications accurately
- Communicate patient emergent needs to primary care clinicians
- Keep Team Supervisor informed of pertinent care plan issues
- Coordinate across cross-organizational areas to meet patient needs
- Adhere to documentation protocols and best practices
