Requirements
- Professional degree (RN, BSN, LCSW, or MSW)
- Experience in acute care, home health, or complex care coordination
- Proven track record of managing high-acuity cases
- Ability to make critical decisions under pressure
- Tech-native, leveraging data and digital tools
- Ability to advocate for patients to prevent hospital readmissions or secure denied services
- Primary internet connection with a minimum speed of 15 Mbps
- Backup internet connection with at least 10 Mbps
- Desktop or laptop with Intel Core i5 (8th generation or newer), Intel Core i3 (10th generation or newer), AMD Ryzen 5, or equivalent processor
- Minimum of 8 GB RAM
- Backup device meeting Intel Core i3 processor performance
- Functioning webcam
- Noise-canceling USB headset
- Quiet, dedicated home office space
- Smartphone for communication and verification purposes
Responsibilities
- Bridge the gap between clinical intent and actual patient outcomes
- Ensure every patient follows an optimized, evidence-based trajectory
- Preempt complications before they require emergency intervention
- Identify and dismantle bottlenecks in the referral and authorization process
- Curate and deploy a bespoke network of community, clinical, and financial resources for each case
- Act as the single source of truth between doctors, insurers, and families
- Ensure 100% alignment on the care objective
