Requirements:
Current State License as a Registered Nurse 2 years of home care experience as a registered nurse in a Home Health or Hospice environment 1 year of supervisory experience Advanced nursing skills Ability to manage patient care episodes, changes to plan of care, clinical evaluations, and recommendations Ability to set objectives, assess staff competencies, allocate work, facilitate team alignment, and create a motivating, safe working environment Ability to provide constructive feedback and develop clinical quality, core competencies, and productivity Ability to seek patient feedback, advocate for patients, understand patient needs, and manage patient expectations Proficiency in enterprise applications (HC Net) Ability to convey information and opinions, listen, check for understanding, and respond appropriately Ability to develop trusted relationships and networks Valid driver’s license Reliable transportation to perform job tasks
Responsibilities:
Provide day-to-day support to direct care team members Supervise and oversee patient’s plan of care Achieve clinical and administrative outcomes Ensure safe, cost-effective services Coordinate care during home health benefit period Manage staff engagement and communication Oversee safety and training of staff Operationally lead and develop a team of healthcare providers Utilize agency policies and procedures Create policy if needed with approval Oversee day-to-day operations of regional staff Provide regional coverage in the field as needed Support and promote quality patient care Ensure adequate supervision through re-evaluating patient needs and home visits Observe and assist with implementation of safety and infection control policies Assure accuracy of OASIS assessments Review, sign, and fax 485’s and verbal orders to MD Serve as a liaison between patient and disciplines Coordinate/supervise care with patient care team Report patient treatment reactions and condition changes to physician Supervise patient teaching, counseling, and supervision Facilitate in-service training programs and client case conferences Ensure compliance with state and federal laws and regulations Receive and assist in resolving regional patient complaints Provide daily operational oversight of area Home Health team Collaborate with multi-departments within the business line Oversee clinical outcomes and KPIs Delegate tasks for efficient region operation Serve as mentor and local engagement leader for care team members
