Requirements:
RN compact multi-state license At least 1 additional state license (CA, MI, IL, or AK) Willingness to expand state licensure if needed 3+ years of experience treating patients with chronic conditions (CHF, Hypertension, and/or Diabetes) Experience in an outpatient or inpatient setting
Responsibilities:
Respond to inbound patient clinical questions and escalations by phone. Follow standardized triage protocols and escalate appropriately. Support clinical documentation for the patient's Cadence care team. Follow up with patients with abnormal readings. Monitor patient vitals, symptoms, and labs for clinical interventions. Perform administrative support as needed.
