Requirements:
Must possess an active LPN license in Virginia Demonstrated strong clinical judgment combined with excellent verbal communication and phone skills Highly organized, proficient with technology, and adept at managing simultaneous digital platforms Prior experience in Remote Patient Monitoring (RPM), telehealth, or complex care coordination is highly desirable A deep commitment to preventive care and reducing hospital readmissions/ER utilization Ability to thrive in a structured, fast-paced in-office team environment Minimum Technical and Work Environment Requirements: Internet Connection: Primary internet connection with a minimum speed of 15 Mbps. Backup internet connection with at least 10 Mbps. Backup connection must be capable of supporting work during a power outage. Primary Device: Desktop or laptop equipped with at least: Intel Core i5 (8th generation or newer), Intel Core i3 (10th generation or newer), AMD Ryzen 5, or an equivalent processor. A minimum of 8 GB RAM.
Responsibilities:
Proactively manage patient health by monitoring remote patient monitoring (RPM) dashboards and responding to vital sign alerts Conduct regular outreach calls to patients to assess symptoms and encourage consistent daily device usage Coordinate timely follow-up care with Nurse Practitioners (NPs), Primary Care Providers (PCPs), or patient caregivers Maintain accurate documentation of all patient encounters within the EMR and internal tracking systems Ensure patient compliance and smooth operation by troubleshooting remote monitoring device issues Act as the primary clinical point of contact for an assigned panel of remote care patients Assist with patient enrollment and onboarding activities as needed to support growth Collaborate daily with the clinical team (NPs), intake staff, and support personnel
