Requirements
- Must possess an active LPN license in Virginia
- Demonstrated strong clinical judgment combined with excellent verbal communication and phone skills
- Highly organized, proficient with technology, and adept at managing simultaneous digital platforms
- Prior experience in Remote Patient Monitoring (RPM), telehealth, or complex care coordination is highly desirable
- A deep commitment to preventive care and reducing hospital readmissions/ER utilization
- Ability to thrive in a structured, fast-paced in-office team environment
- Minimum Technical and Work Environment Requirements: Internet Connection: Primary internet connection with a minimum speed of 15 Mbps. Backup internet connection with at least 10 Mbps. Backup connection must be capable of supporting work during a power outage.
- Primary Device: Desktop or laptop equipped with at least: Intel Core i5 (8th generation or newer), Intel Core i3 (10th generation or newer), AMD Ryzen 5, or an equivalent processor. A minimum of 8 GB RAM.
Responsibilities
- Proactively manage patient health by monitoring remote patient monitoring (RPM) dashboards and responding to vital sign alerts
- Conduct regular outreach calls to patients to assess symptoms and encourage consistent daily device usage
- Coordinate timely follow-up care with Nurse Practitioners (NPs), Primary Care Providers (PCPs), or patient caregivers
- Maintain accurate documentation of all patient encounters within the EMR and internal tracking systems
- Ensure patient compliance and smooth operation by troubleshooting remote monitoring device issues
- Act as the primary clinical point of contact for an assigned panel of remote care patients
- Assist with patient enrollment and onboarding activities as needed to support growth
- Collaborate daily with the clinical team (NPs), intake staff, and support personnel
