Travel Primary Care Nurse Practitioner/Physician’s Assistant 

Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license State-specific license in good standing in Pennsylvania Minimum 6 months of experience in long-term care or a skilled nursing facility Strong leadership, communication, and team-building skills In-depth knowledge of regulatory requirements and quality improvement processes in long-term care Ability to collaborate with interdisciplinary teams and external stakeholders Experience in telehealth or virtual care settings (preferred) Strong clinical judgment, communication, and patient engagement skills Proficiency with electronic medical records (EMR) and telehealth technology

Provide high-quality telehealth primary care services Conduct virtual assessments, diagnose, and develop treatment plans Manage chronic diseases and acute conditions through telemedicine Collaborate with interdisciplinary teams Maintain accurate medical records Assist with quality management (QM) initiatives Ensure adherence to regulatory guidelines Provide guidance and support to healthcare team members Occasional travel to Long Term Care facilities for in-person care
