Requirements:
Active unrestricted licensure as LCSW or RN in Florida. Willingness to obtain licenses in additional states as needed. For RNs, Bachelor of Science Degree in Nursing. Minimum 4 years of experience in case management, care navigation, and patient advocacy. Minimum 2 years of supervisory or leadership experience in a healthcare environment. Extensive knowledge of community-based resources and social service programs. Excitement for a fast-paced, high-growth, feedback-oriented environment. Access to a quiet, HIPAA-compliant, distraction-free workspace with a hardwired network connection. Experience in community health, primary care, FQHCs, or home health settings is valued. Understanding of Medicare and Medicaid coverage is preferred. High proficiency in Spanish is preferred. Outstanding communication and interpersonal skills. Exceptional organizational skills and self-directed time management. Strong detail-orientation. Ability to multitask and operate several applications simultaneously. Critical thinking and problem-solving abilities. Proactive and comfortable working without direct supervision. Ability and willingness to adapt to changing work environments. Highly tech-savvy and able to learn new systems quickly. Desire to coach future Care Advocates.
Responsibilities:
Build trusting relationships with Medicare members. Manage member communications via calls, video, text, and email. Connect members to community-based social services. Support appointment scheduling and healthcare access. Facilitate behavior change, provide emotional support, and offer health education. Participate in an on-call schedule for urgent needs. Assess and prioritize member needs. Develop care plans addressing social determinants of health. Document member interactions and support activities. Maintain a database of community resources. Communicate with practitioners and healthcare facilities. Translate case management experience to improve team processes. Partner with manager to improve SOPs and workflows. Develop onboarding and training materials for new Care Advocates. Serve as a touchpoint for questions from Care Advocates. Implement pilot projects or process improvements. Represent Papa Community at in-person events.
