Remote Oncology Nurse Navigator (2:30 - 11 PM EST Shift)

A Bachelor of Science Degree in Nursing and a compact unrestricted registered nurse (RN) license are required. Compact + single state licensure in Alaska, New York, Illinois, Michigan, and/or California licensure is preferred. At least 5 years of nursing experience, including 3 years in solid tumor oncology nursing. Oncology-related certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) are required or obtained within 2 years of hire.

Conduct comprehensive clinical assessments. Oversee member health. Facilitate end-of-life care discussions. Engage with members, addressing clinical issues, and managing escalations. Familiarize yourself with Thyme Care systems, tools, technology, and partners. Collaborate with Nurse leaders and Medical Directors. Establish trusting relationships with members and their care network. Adhere to Care Team policies, procedures, and documentation standards. Support members throughout the oncology care continuum. Identify and address member needs promptly. Participate in case conferences. Foster strong partnerships with payers and providers. Collaborate with non-clinical Care Team members. Be available for urgent clinical escalations. Performs other projects and duties as assigned and as related to department business needs and objectives.
