- Carry a flexible caseload of members (up to 29hrs/week) and complete intakes
- Attend and respond to multiple tasks simultaneously
- Maintain accurate and timely documentation
- Collaborate with member's Clinical Team
- Provide coaching support around various topics
- Collect information on risk and escalate care as needed
