- Develop the clinical framework for building and growing the CookUnity RDN network, including provider competencies and recruitment and vetting processes.
- Co-develop evidence-based nutrition protocols for diabetes, oncology, weight management with GLP-1s, and cardiovascular conditions.
- Define nutrition assessment tools, flagging criteria, and escalation pathways for medically complex members.
- Build structured frameworks for nutrition education and counseling visits.
- Integrate Motivational Interviewing, Stages of Change, and SMART goal-setting into care standards.
- Develop clinical documentation standards, quality metrics, outcome tracking, and a clinical resource library.
- Conduct nutrition assessments, create individualized care plans, and deliver nutrition counseling and education to members.
- Collaborate with health plan care teams and care coordinators on member care management and referrals.
- Provide clinical oversight, conduct chart audits and case reviews, and facilitate peer review sessions for the RDN network.
- Lead RDN onboarding and training and identify continuing education and skill-building needs.
