Job Details

Previous experience in care coordination, service delivery, scheduling, or administrative support within aged care, disability, or community services.
Proficiency in scheduling software, CRM systems, and Microsoft Office tools.
Strong computer skills and willingness to learn.
Strong organizational and time management skills, including the ability to manage multiple priorities.
Excellent communication and interpersonal skills with a client-focused approach.
High attention to detail, accuracy, and documentation management.
Ability to work independently while collaborating with a wider support team.
Knowledge of care compliance standards and client confidentiality requirements.
Empathetic and professional approach when working with clients and caregivers.
Problem-solving mindset and proactive approach to challenges.

Coordinate the scheduling and delivery of home care services in line with client care plans.
Serve as the primary point of contact for clients, their families, and support workers.
Match clients with suitable caregivers and update schedules as needed.
Manage service delivery coordination and rostering.
Perform data entry and compliance checks, and maintain accurate client records, care plans, and documentation.
Follow up on client feedback and assist in resolving service-related concerns.
Collaborate with the care team on service standards, safety protocols, and regulations.
Provide invoice and accounts receivable updates, and assist with caregiver and staff onboarding.
Support internal reporting, compliance efforts, content updates, and marketing tasks as directed.
