Requirements
- Be a licensed nurse or MD.
- Have advanced English proficiency at C1 level or higher.
- Have proven experience working with U.S.-based medical offices.
- Have experience in chronic care management, patient follow-up, or care coordination.
- Demonstrate clinical judgment and the ability to assess patient conditions remotely.
- Have experience providing patient education and support.
- Be highly organized and have strong documentation skills.
- Be comfortable working independently in a remote environment.
- Have a reliable computer with at least 8GB RAM.
- Have a stable internet connection of at least 100 Mbps.
- Have a quiet, professional workspace and a headset with microphone.
- Work 45 hours per week, Monday through Friday, during the stated Florida-time schedule.
Responsibilities
- Maintain ongoing communication with patients enrolled in the PCM program.
- Monitor patient symptoms, medication adherence, and progress and response to treatment.
- Provide patient education about their condition and care plan.
- Assist with implementing and following up on care plans.
- Coordinate services and ensure continuity of care.
- Identify changes in patient status and escalate concerns to the provider when needed.
- Support patients in following treatment recommendations and keeping scheduled appointments.
- Document patient interactions and updates in the system.
