Requirements

  • Hold an active California Registered Nurse (RN) license in good standing.
  • Have an associate's, bachelor's, or master's degree in nursing.
  • Demonstrate proficiency with multiple technology platforms and electronic health record (EHR) systems.
  • Be bilingual in English and Spanish.
  • Be able to manage competing priorities and collaborate in a remote, virtual environment.
  • Be comfortable working with diverse, medically and socially complex pediatric populations.
  • Have reliable internet access and a dedicated workspace suitable for remote work.
  • Work Monday through Friday, 9 AM to 6 PM PST, with a one-hour daily break; occasional Saturday coverage may be scheduled in advance.
  • Preferred: experience in pediatric, chronic care, or care coordination settings.
  • Preferred: familiarity with prior authorization processes and payer requirements.
  • Preferred: experience working in underserved or urban environments.

Responsibilities

  • Coordinate, schedule, and reschedule clinical appointments across the care team.
  • Manage referrals to specialists and external clinical services, and follow up with families to reduce gaps in care and no-show rates.
  • Support care transitions and continuity across the interdisciplinary team.
  • Provide clinical updates to referring physicians and coordinate lab work and documentation for treatment planning.
  • Provide evidence-based education to families about chronic condition management, medication regimens, treatment plans, and care goals.
  • Respond to patient and family inquiries through messaging and phone.
  • Initiate and manage medication prior authorization requests, coordinate with pharmacies, track authorization status, and document communications.
  • Document clinical interactions across EHR and care management platforms and participate in case conferences, team huddles, and quality improvement meetings.
  • Report patient progress and care gaps to clinical supervisors and contribute to workflow improvements.
